Who can make the referral?

  A parent or family member
  A family physician, pediatrician or other healthcare provider
  Case managers, social workers or mental health providers
  Counselors, teachers

Who should I contact at the Aaron Center?

Contact us at (570) 489-5561 extension 10 or at gyanick@aaroncenter.com to let us know that you are referring for a service. That individual will then provide direction and explain the process for obtaining services.
 

To finalize the referral process, you will need to provide:

  Name of individual or family
  Client’s social security number and subscriber’s social security number
  Name of primary care physician
  Insurance information located on the front and back of the insurance card

Please have this information available when you contact the Aaron Center. Once we receive this information, we will obtain the necessary authorizations and prescriptions. If your physician provides you with a written prescription, please remember to bring it with you at the time of your appointment
